This year, information waspresented on Alzheimer’s disease, amyotrophic lateral sclerosis, epilepsy, myasthenia gravis, headache andmigraine, Huntington’s disease, multiple sclerosis, narcolepsy, Parkinson’s disease,tics and Tourettesyndrome, and more. Alzheimer'sDisease Lecanemab for the Treatment of Early Alzheimer’s Disease: The Extension ofEfficacy Results from Clarity AD Michael Irizarry, MD(Eisai) Dr. Michael Irizarry presented results from the tau PET substudy from lecanemab’s Phase III Clarity AD trial,as well as results from the open-label extension through two years. Tau PET was measured at baseline and followed up in 342 subjects. Lecanemab slowed the spread of tautangles in several cortical regions, but especially in the early Braak regions, particularly the medial temporallobe, meta temporal lobe, and temporal lobe. 40% of participants in the tau PET substudy were classified as having low tau, with tau restricted to theentorhinal and transentorhinal cortex. Although this low tau subgroup is associated with slower progression,separation is nonetheless seen from placebo on cognitive outcomes, like CDR-SB. From 18 to 24 months,separation is maintained between the early-start arm that initiated lecanemab at thebeginning of the study and the delayed-start arm that started treatment at 18 months, suggesting a benefitto starting treatment earlier. Additionally, in comparison to a natural history cohort, as the extension periodwas open label, the delayed-start group separated from matched controls. Clinical Relevance of Donanemab Treatment AlirezaAtri, MD, PhD(Banner Sun Health Research Institute) In a post-hoc exploratory analysis, Dr.Atri from the Banner Sun Health Research Institute showed datademonstrating a significant beneficial effect of donanemab on more interpretable individual itemmeasuresfrom iADRS and CDR-SB domains. Specifically, donanemab improved specific memory items, like keepingappointments or talking about something the patient saw on TV, or items related to executive function likemaking a meal, using household appliances, and personal care, thus enabling greater independencefor alonger period of time. Donanemab treatment delivers outcomes that are valued by patients and their care partners, includingbenefits seen across several individual cognitive ADAS-Cog and functional ADCS-iADL items and across allsixdomains of the CDR. CDR-SBdomains Dr. Atri showed a Venn diagram outlining outcomes valued by different stakeholders in the Alzheimer’slandscape, including patients, their care partners, and healthcare professionals.
